Informed consent is always obtained from participants before collecting personal information. This means that Campus DBT will never collect any personal information without your knowledge and permission.
How your personal information is kept confidential
Where it is necessary to gather personal information, data is kept confidential. Personal information is kept in secure files and access is restricted. Additionally,Campus DBT makes reasonable efforts to protect your personal information from loss, theft, unauthorized access, disclosure, copying, misuse and alteration.
Other ways personal information may be obtained and how it is protected
“Contact Us” Form: The Campus DBT website contains a Contact Us page so visitors can ask specific questions of interest to them. The Contact Us page contains an email form so you can comment, make suggestions, request information and ask questions. On this form, your first name and email address are required so we can answer your questions, however the “last name” field of the contact form is optional – you do not have to provide your last name in this correspondence. Note that we may also track your browser type and referring URL to better diagnose technical issues you may be having. Information sent via the Contact Us page is not shared with third parties. It is simply used to answer your correspondence.
Email: Campus DBT does utilize a HIPAA compliant email system. However, electronic communication does come with its own set of risks.
Withdrawing your consent to provide personal information
If you have provided Campus DBT with personal information in the past and then have second thoughts or want to update it, please contact us. Please note that it is not always possible to remove or modify personal information in the database, but we will make reasonable efforts to do so upon request.
